Management: Conduct Grade Sheet?

Hi

I'm looking for some type of weekly conduct grade sheet that is sent home for parents to sign. Currently my parents have just been signing the newsletter that lists the conduct grade, but as the year has progressed I really don't like this anymore. I want the students to be more responsible for their own grade and take responsibility so I'm looking to change my system, yet I'm not 100% sure of exactly what I want. Is anyone willing to share their templates of what you use? Btw, I teach 5th grade.

Here is the one I use

I send this home every week in our Friday Folder. I stuff all the work to be sent home for the week and then jot in a conduct and work habits grade. Very quick, well when I have students file the work to be stuffed first!

This one is adapted from Ms. Powell's site--I also send home weekly grades since I have everything on my computer anyway.

glad you could use it! it works wonderfully. forgot to mention that we have fun centers in friday and if my kids have more than 3 marks on the chart they have to sit out 2 minutes of centers per mark. does that make sense,lol!
