Does anyone have a Reading Interest Survey that you just love? I've googled and looked at past posts, but am still looking for ideas. I just thought I'd ask if anyone had a one they are currently using and wouldn't mind sharing.

Good questions on the survey Lynn6 posted. I also add a question about whether their parents read aloud to them and whether their parents read (or if the kids have even noticed this....very telling!)
