writing chart help

Help please! I'm trying to find an anchor chart for K writer's workshop. It had 6-7 categories... e.g. I am working on leaving space between words.... I am working on starting sentences with capital letters... etc.... followed by a space for kids' names on post-its. This site had printables ready to download.

If anyone can help, I'd appreciate it very much!
